Copy your session notes from SimplePractice AI or your own notes and paste them in the left panel. The more detail you provide, the better the formatted output.

Enter the client initials, session date, therapy type, and ICD-10 diagnosis. These are injected directly into the formatted note. If you do not have the exact ICD-10 code, enter your best guess, you can review it in the output.
Select the note type that matches your session (see the "4 Note Types" tab for guidance). Click Format Notes. The formatted note appears on the right in 5–15 seconds.
Read the output carefully. Look for any [VERIFY] tags, those need your attention. Run the Compliance Check, address any flags, then Copy and paste into SimplePractice.
Always review before saving
AI-formatted notes are a starting point, not a final document. You are the clinician of record. Verify all clinical content before it goes into the client file.
Use for: First session with a new client
Covers informed consent, GFE, biopsychosocial assessment, suicidal ideation screening, mental status exam, and initial plan. This is your intake note.
Use for: All regular sessions after the intake
Formatted for insurance submission. Covers medical necessity, session focus, interventions, client engagement, risk assessment, and plan. Includes ICD-10 and treatment goal placeholders.
Use for: Creating or updating the formal treatment plan
Generates SMART objectives with target dates within 3 months, a required review date, evidence-based interventions, and signature lines. Must be completed within 3 sessions per Section 11.4 compliance.
Use for: Any session where EMDR processing was the primary modality
Covers EMDR phases completed, SUD/VOC ratings if documented, target memory or belief, medical necessity for trauma-focused therapy, and next session plan. Include SUD/VOC numbers in your raw notes for best results.
When the Auto de-identify checkbox is on, the tool scans your raw notes for patterns that look like full names (First Last format) and replaces them with initials (e.g. "Jennifer Smith" becomes "J.S.") before anything is sent to the AI.
Always double-check the output
Auto de-identification catches most full names but is not 100% reliable. Always read the formatted note before copying it into SimplePractice. If you see a full name in the output, edit it to initials manually.
After formatting, click Compliance Check to automatically scan the output for common issues:
Address all red flags before filing. Review amber flags and use your clinical judgment.
Nothing happens when I click Format Notes
Your session may have timed out or the server endpoint may be temporarily unavailable. Try again in a moment.
I get an API error message
If this persists, contact support; the server endpoint may be rate-limited or misconfigured.
The output is missing key sections
Your raw notes may not have included enough detail for that section. The AI can only format what you give it. Add more context and reformat.
I am locked out after failed password attempts
The lockout lasts 5 minutes. After that, try again. If you have forgotten the password, contact Mark at mark@wynstudios.com to reset it.
